the map page of this website to ensure your
home address is within our practice area. Please bring your medical card to the
surgery. If you cannot find your card, bring proof of identity, proof of address
and your NHS number and you will be given an application form and a medical questionnaire
to fill in. You must make an appointment with the nurse for a health check.
